By enrolling in the Annual Membership Program at Dental Avenue, you agree to the following terms and conditions:
Non-Refundable & Non-Transferable
- a. The membership fee paid is strictly non-refundable, regardless of usage, circumstances, or discontinuation.
- b. Membership is non-transferable to any other individual, including family members or friends.
Membership Validity
- a. The membership is valid for a period of 12 months from the date of enrolment, unless otherwise specified under your plan.
- b. Renewals must be done prior to expiry to continue enjoying uninterrupted benefits.
Eligibility & Clinical Discretion
- a. Certain dental procedures and preventive treatments are performed only when clinically indicated.
- b. For example, fillings are provided only in the presence of a confirmed cavity. In absence of clinical need, such procedures cannot and will not be performed solely under the membership benefit.
- c. The clinical team retains full professional discretion to determine the suitability of any procedure for a member.
Usage Limits & Benefit Tracking
- a. Each plan has clearly defined inclusions, exclusions, and limits (e.g., number of cleanings, fluoride applications, consultations, etc.).
- b. Benefits must be used within the validity period and do not carry forward to the next membership cycle.
- c. No monetary exchange or reimbursement will be offered in lieu of unused benefits.
Identification Requirement
- a. Membership privileges can be availed only after proper verification using the registered mobile number, patient ID, or membership ID.
- b. Failure to provide identification may result in denial of membership benefits for that visit.
Procedure Limitations
- a. Membership covers preventive and selected routine procedures only, as specified in the membership brochure.
- b. Advanced treatments, cosmetic services, or lab work (e.g., crowns, aligners, veneers, implants) are not covered unless explicitly mentioned and will be charged separately.
- c. Any procedure not included in your membership plan is chargeable at prevailing clinic rates.
Conduct & Cooperation
- a. Patients are expected to maintain reasonable appointment discipline, follow clinical instructions, and cooperate during visits.
Data Privacy
- a. Your personal data collected during membership enrollment is stored securely and used only for clinic communication, treatment, and tracking of benefits.
- b. We do not share your data with third parties without your consent, except as required by law.
Modification & Termination
- a. Dental Avenue reserves the right to modify membership benefits, pricing, or terms with prior notice.
- b. The clinic may suspend or terminate a membership in case of misuse, fraud, or violation of these terms.
Jurisdiction
- a. All disputes arising out of or in connection with this membership shall be subject to the jurisdiction of courts in Bangalore, India.
